An investigation into the deaths of two port workers within six days of each other has found major safety issues within the stevedoring industry.
Atiroa Tuaiti from Auckland and Don Grant from Lyttelton both died in April last year.
In a report released today, the Transport Accident Investigation Commission found a haphazard approach to safety, with poor regulatory oversight and a lack of industry-wide standards.
