Top officials were kept in the dark about a bungled $40 million Oranga Tamariki project to expand services to children who are victims of sexual abuse.
The Government says what went on is an "incredibly serious" failure and it's demanding better from the agency's bosses.
The Children's Commissioner only got assurances the work was back on track last week, on the same day RNZ asked her about it.
