Prison assault wasn't followed up: inquest

An inquest has heard a man who died after a suspected assault in Mt Eden prison had told authorities and medical staff, but it wasn't followed up or tracked. 25-year-old Nick Evans, who served time for drug offending, died suddenly in Whangarei Hospital in June 2015. His death was one of a number of headline-grabbing incidents at Mt Eden, which culminated in the Government deciding not to renew private operator Serco's contract.
