A Health and Disability Commissioner report found the Hutt Valley District Health Board its staff provided substandard care and breached the Code of Rights in the case of a baby that died in 2016.
Ruth and Kazu Toyoshima's girl Ellie died after she was deprived of oxygen and suffered brain damage during birth.
