Briefings to the Government say a key department had not identified critical services or tested staff ability to work from home as Covid-19 caused havoc earlier this year.
The briefings also say 'insufficient' computer system capacity presented a 'very high risk' of an 'ineffective' pandemic response.
The Ministry of Business, Innovation and Employment says its IT staff did a 'stunning' job.
