Children's Commissioner Judge Frances Eivers says it should be mandatory for Oranga Tamariki to investigate every report of suspected child abuse.
It comes after the Chief Ombudsman released a scathing report saying the agency failed to take action when concerns were raised about the safety of five-year-old Malachi Subecz, who was last year murdered by his carer.
Malachi's whānau raised concerns for months but nothing was done by the state agency.
