An independent review into the death and murder of five-year-old Malachi Subecz has proposed major changes - including making it mandatory for anyone tasked with child protection to report suspected abuse.
The Government has approved nine of the recommendations, but won't yet commit to the remaining five. That includes compulsory reporting.
