The Corrections Association says an Auckland jail should have been locked down when two men rampaged on Sunday.
Prisoners at Mt Eden Corrections Facility smashed a ping-pong table and tried to break glass to get into a guardroom.
A stand-off with Corrections officers went on for about three hours. Meanwhile, a third prisoner started a small fire inside his cell.
The association says Corrections failed to call a Code Red.
