Almost 3000 of the country's most vulnerable children are not enrolled with a doctor - and GPs say Oranga Tamariki has never asked them for help to fix that.
They say inmates released from prison and refugees can get help finding a doctor, so why not children in state care.
Phil Pennington reports.
Oranga Tamariki does not dispute the figures in the monitor's report
But it says being enrolled with a GP is only one measure.
It says its files show 88 percent of children in care are having their health needs assessed and have a plan to address them
Both it and the ministry say this is a top priority and they are working on an action plan so agencies cooperate more to deliver the care.
