Worksafe, the workplace safety watchdog, never investigated the death of two temp workers despite carrying out research into the labour hire industry.
The two men, Jake Ginders and Floyd Harris, were on the way to a job when they died in a road smash in early 2019.
The driver Floyd Harris only had a learner license.
