Two investigations have begun into the death of a teenager at a Palmerston North IHC support centre.
Nathan Booker apparently drowned in a bathtub at the Woburn Family Support Centre on 10 January.
The centre is operated by IDEA Services, which is owned by the IHC.
Police are investigating the 15-year-old's death and Detective Senior Sergeant Craig Sheridan said everyone involved in the boy's care will be interviewed.
Mr Sheridan said the family of the special-needs boy are devastated.
The IHC has also begun its inquiry. Spokesperson Paula McArthur said what happens next will be determined by the results of the inquiries. Staff involved in the teenager's care will receive counselling.
The funeral will be held on Wednesday.
