A man who was released from prison, managed to leave the country last month without alerting authorities - the second case to come to light in a week.
The man was released from an Auckland jail in September and flew to Australia in October, breaching his release conditions.
Corrections Department head Ray Smith said the man legally obtained his passport, but was not supposed to travel overseas.
He said this latest case will be included in the multi-agency review which was announced last week after another offender, Phillip Smith, managed to flee to South America, while on temporary release.
The review will look at information sharing between agencies.
Police have issued a warrant for the man's arrest and are working with Australian authorities to bring him back to New Zealand.
