Request for new coroner's investigation highly unusual - QC

A coroner's decision last year ruled that Nicky Steven's death was a preventable suicide, after he walked out of the Waikato Hospital's mental health unit and was found dead three days later. The Waikato DHB has now requested the Solicitor-General open a new inquiry, a decision that has left Nicky's parents reeling.
