A mental health respite centre in a Porirua neighbourhood is to be moved, after a patient tried to abduct a child.
In March, the 32-year-old man tried to grab a 3-year-old girl who was with her father.
A report, released today, said a less experienced staff member was in charge of assessing the man, and he was put in a facility that was not suitable.
It also said the level of staffing at the respite house was below a safe level.
Capital and Coast DHB spokesperson Nigel Fairley said the centre was not big enough to take the changes recommended in the report.
"So the decision has been made to move it. We will do that in a systematic and thoughtful way, so there is no timeframe around that."
Mr Fairley said staffing at the respite centre had been increased.
